Having had a star section at 32 weeks because of some COVID related issues , she wanted no intervention and I respected that . She had a 2 page birth plan and a doula and I tried to simulate a home delivery with her doula and husband. You cannot imagine the administrative shit that went on because … and I’ll say it was nerve racking … the 10 hours of screaming and refusing to be checked . That to say , she let me get an IV in case if emergency but that’s all I asked . The Chief of OB called me twice Witt complaints from the nurses because she didn’t want pitocin , erythromycin ointment etc as was her birth plan . Then … risk management to sign an against medical advice . We had a natural delivery peppered with some exorcist type moments but she had a healthy baby , also refusing the vitamin K and the hep b which I was fine with . That to say , as I get back from another midnight delivery , that as I looked up at her as I have’s her her baby , I was just appalled by all the stuff still connected … the heart monitor psds , the pulse ox , iv tubing , and the blood pressure cuff hyperinflating while she’s trying to do skin to skin. It’s too much .
We do have an increase in home births and for most it’s fine but we really do have a higher risk population ( like under 240
Is “ normal weight “ when signing out so it’s more of a challenge .
Ironically , the ones with the birth plans at 36 weeks are wearing me down by 39 weeks with repetitive triage visits and then looking for all kinds of reasons to need to be induced and if they do end up getting an epidural , screaming because of the “ ring of fire “ as the crown ( what I just had ). I think our population has no idea that’s it’s weird to have to wait in line at CVS behind the 12 car line to get your 10 meds or that at 37 , it’s weird that you are on 17 and need a knee scope .
I’m living in the wrong time I think .

It’s the most frustrating time of my life and I actually give a shit. All my patients have my number and text me there or on Facebook messenger . I’m solo boutique in a Dollar General town and becoming super burned out with all the BS. 25 years in to it and I can’t even believe how different my practice is compared to even 7 years ago . It’s not fun. It’s a struggle all the time. I finish my 47 patient clinic and then chart for 4 hours to send lifestyle info in there , chart enough to justify my labs and tests , and send podcasts to the ones that seem to care . I guess I’m just venting because the way you approach medicine and people is the way my dad would have loved ( and he would have loved your style ) and I’m anything but. Just a cog in the wheel .
